Acknowledgement of Risk
I acknowledge that participation in self defence, karate, and martial arts involves inherent physical risks, including but not limited to muscle strains, bruising, sprains, fractures, concussion, serious injury, or death. I understand that these risks may occur despite proper instruction and supervision.
Voluntary Participation & Fitness
I confirm that participation is voluntary and that I (or my child) am physically and medically fit to participate. I agree to follow all instructor directions and safety rules and to advise the instructor of any medical condition or injury that may affect safe participation.
Assumption of Risk & Release
To the fullest extent permitted by law, I assume all risks associated with participation and release and waive all claims against Karate Community, its instructors, staff, volunteers, and representatives for any injury, loss, or damage arising from participation, including claims based on negligence, except where liability cannot be excluded under Australian law.
Australian Consumer Law
This waiver operates subject to the Competition and Consumer Act 2010 (Cth) and relevant Queensland Civil Liability legislation, and does not exclude rights that cannot be lawfully excluded.
Medical Treatment
I authorise the instructor or staff to obtain medical treatment if required in an emergency and accept responsibility for any associated costs.
